Name: Lake Hylia Temple
Description: An ancient temple built long before Ganon's war by an ancient Zora King.  Various other fortifications were built throughout the Zora's civilization, many of which were destroyed over the years.  The temple itself was greatly damaged in Ganon's War and has been reconstructed over the years, although many parts inside are still damaged.  The pressurized system of the temple is maintained by an internal engine that makes its way throughout the entire temple.  The cog is actually part of the core of this engine.  This was done to make it impossible to remove it from the temple, however there is a way to keep the engine running without the cog.

Items at this point:
Boomerang
Hookshot
Flippers
Magnetic Gloves
Forest Cog

Floors:
Floor 1, above ground
Floor B1, submerged level
Floor B2, subterranean level
Floor B3, Grave level (Boss at this level)
Floor b4, Cog Chamber

Puzzle Types:
Changing pressure levels to access different areas (by moving water out of the way or by raising platforms for example)
Time events where pressure becomes too high or low in a room
Use polarity to cross gaps or activate certain switches
Shutting down various systems temporarily using the hookshot or boomerang
Ledge Hanging and Wall running to get through certain areas
Using flippers to traverse water filled areas
Pushing opposite polarity items into/onto a switch

What about using the magnet gloves to climb arcoss certain walls?

Changing the water pressure sounds like a sufficiently nightmare-inducing mechanic for a water dungeon.  ;)

Alright, here's the first floor so far.  Lemme explain how this will work...

First, the map key. 

Black straight lines are doors.
Red straight lines are locked doors (key)
Dark blue is diving water
lighter blue is swimming water
lightest blue is shallow water
yellow balls are special switches
green boxes are puzzle elements
brown boxes are chests
red boxes with black outline are locked hatches to the floor below
grey lines are ledges, a gap represents stairs

This is the first floor only.

So you'll enter in that little room in the bottom middle and proceed to the main room (yes I know I forgot a door there.  There should be one).  You'll go to the door on the left where you'll encounter an enemy (don't know what yet).  Proceed to the door below.  Come down the stairs and enter the door on the right.  Fight off some enemies for a key (very basic enemies, again don't know what yet).  Back in the previous room, unlock the door and engage the pressure switch by pulling it.  A geyser will appear infront of the player to show that something has changed.  This removes the pressure from part of the floor below.  Backtrack to the main room and dive into the pool at the top to enter the next hallway.  Another small enemy here, go to the door to the left.  Hit the boomerang switch in the middle, causing 4 blocks to fall.  The player must push the blocks together to form an image of the Zora's Sapphire in 60 seconds.  Similar to the poe puzzle in OoT's forest temple.  This opens the hatch in the caged room below.  The other hatch is opened when the pressure switch from earlier is open.  Entering the upper hatch, they'll be brought to the floor below to do something (don't know what yet) to get to the other hatch.  Using a ladder to come up, you'll grab that key and backtrack to the previous hallway where you can now unlock the door.  Not sure what's gonna come after that, but keep checking back!

It's a bit hard to follow right now with only one floor to look at when other floors keep getting referenced xD.  One thing I noticed though, is that the player seems to be clearing the entire first floor before doing much on the lower floors.  Is the rest of the dungeon going to follow that pattern?

I'm not sure yet, I'm improvising as I go along.  There will be backtracking though, I can assure you of that :)

I think ice is cool. Can the bottom floors be iced over because of some malfunction, perhaps some cogs not working correctly, fallen out, perhaps because of the boss causing havoc?

Like the idea with ice, ice also takes more space then water, so perhaps some areas are unreachable unless frozen. Also you might be required to freeze water inorder to push an object, or place an object in a specific area on the ice, where if the ice was defrozen it could fall on a switch (or create a platform)
