So what happened to Hyrule? Did the goddesses shatter the world due to the races battling against each other? The title and the story feels a bit generic. It's not a bad thing though. But I always believe the story of a game helps make it stand out. I'm assuming there will be either a hub area, where all the portals connect (maybe like a central dungeon?) or this game might have a Mario game layout where you beat the worlds in a certain order. It'd be cool for Link to travel to the fire world just to collect a key or something so he can progress more through the forest world. Helps and improves gameplay and linearity.

Actually you're entirely right! That's actually the root of the game to connect the portals. The dungeons appear in all realms because of the gods. The gods gave Link a trial to prove he is worth for the master sword. That means each realm is connected to the dungeons in a different way. After defeating the boss his reward is a piece of the master sword. Once he gets all the pieces, he needs to go to the tower of the gods to complete one more challenge to be able to forge them together in one last blessing.
Also about the antagonist, he is not Ganon at all, but he is connected to the term "a thief of the dessert has made his greedy wishes out of the gods power!".
